Our Services

Home is Where the Heart Is!

Nursing

A Nurse Case Manager will complete an assessment to determine the level and type of service necessary to meet the client's needs. A care plan is then developed to guide the delivery of service. Our nurse will continue to review and update the plan as necessary to meet the client's needs and can provide any or all of the following:

• On-going visits as needed to assess and monitor the client's progress
• Supervise and evaluate the performance of the clients Home Health Aide or Companion
• Medication management/pre-pour your medications on a fee basis
• Complete a PRI (Patient Review Instrument) and SCREEN. This is an assessment to determine level of care for nursing home placement on a fee basis

Home Health Aides

Our home health aides provide assistance with activities of daily living which may include personal care, meal preparation, laundry, housekeeping, medication reminders and assistance to and from life activities.

Companions

Our companions can accompany a client to appointments and social outings as well as errands, assist with meal preparation, light housekeeping, laundry, help you write letters, read to you or simply visit. TIC also provides one-on-one companionship for residents who require more individual attention in nursing facilities.

Emergency Response Service

An Emergency Response Service for clients who live alone in their own home can be arranged. These services use a small portable call button which acts as an emergency response system for clients who need assistance and cannot get to their telephone.Please call At Home Services, Jewish Senior Life at 585-427-8456 or CLICK HERE for more information.

Payment Arrangements

All services at Traditions in Caring are billed on a fee-for-service basis. Payment options include: private pay, insurance (including long-term care insurance and based on your individual policy), Medicare and Medicaid as offered by a certified home health agency or long-term home health care program sub-contract.
